Fort Worth, TX and surrounding DFW areas
We are seeking experienced and compassionate Registered Nurses (RNs) to provide skilled home health services to patients throughout the Dallas-Fort Worth area on a PRN basis.
This position is ideal for nurses with prior home health experience who are comfortable working independently in the field and managing complex skilled nursing visits.
- Perform comprehensive patient assessments in the home setting
- Complete start-of-care, recertification, resumption-of-care, and discharge assessments
- Develop and implement individualized care plans
- Perform wound care and wound VAC management
- Provide PICC line care, dressing changes, and maintenance
- Administer IV therapy and infusion services as ordered
- Educate patients and caregivers regarding medications, disease management, and treatment plans
- Communicate effectively with physicians, case managers, and interdisciplinary team members
- Complete documentation accurately and timely within agency systems
- Ensure compliance with all Medicare, state, and agency regulations
- Active Texas RN license
- Minimum 1 year of Home Health RN experience
- Current BLS certification
- Reliable transportation and valid driver's license
- Strong assessment and clinical skills
- Ability to work independently in patients' homes
- Experience with Wound VAC therapy
- Experience with PICC line management
- Experience with IV therapy and infusion nursing
- OASIS documentation experience
- Experience with Kinnser/WellSky EMR
- PRN / As Needed
