Job Summary
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The ideal hospice case manager will play a crucial role in coordinating patient care, ensuring that individuals receive the appropriate services and support throughout their healthcare journey. This position requires strong clinical knowledge, excellent communication skills, and the ability to work collaboratively with patients, families, and healthcare providers.
Hospice RN case manager responsibilities
- Conduct comprehensive assessments of patients' needs and develop individualized care plans.
- Coordinate care across various settings, including primary care clinics, emergency medicine, and critical care environments.
- Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
- Collaborate with interdisciplinary teams to facilitate effective communication and continuity of care.
- Maintain accurate medical records and documentation in compliance with regulatory standards.
- Provide education and support to patients and their families regarding treatment options and resources available.
- Advocate for patients’ needs within the healthcare system, ensuring access to necessary services.
- Stay informed about current medical terminology, case management practices, and relevant healthcare regulations.
Hospice RN case manager requirements
- Current Registered Nurse (RN) license in the NM.
- Experience in hospice case management or a related field is preferred; familiarity with geriatrics and end of life care.
- Strong understanding of medical terminology and experience working in hospice (outpatient)
- Excellent organizational skills with the ability to manage multiple cases simultaneously.
- Strong interpersonal skills to effectively communicate with patients, families, and healthcare professionals.
- Ability to work independently as well as part of a collaborative team environment.
- Proficiency in using electronic medical records systems for documentation purposes.
