Job Title: Registered Nurse (RN) - Home Health
Pay: $90 to $130 per visit
Job Type: Per diem (with opportunity to transition to full-time based on company needs)
POSITION SUMMARY
The Home Health Registered Nurse (RN) provides skilled nursing care to patients in their homes under the supervision of a physician. The RN is responsible for implementing start-of-care plans, administering treatments and medications, performing wound care, coordinating discharges and collaborating with the clinical team to achieve positive patient outcomes.
ESSENTIAL FUNCTIONS
- Perform case management and follow-up skilled nursing visits in the patient’s home.
- Conduct patient assessments, consult with physicians, and develop individualized treatment plans.
- Submit regular progress reports to the attending physician and clinical team.
- Coordinate and complete patient discharges in consultation with the physician and clinical manager.
- Deliver direct nursing care and prepare written care plans.
- Educate and support patients and families to ensure care plan compliance.
- Assessing the need for the services of other team members (PT, OT, ST, MSW, and HHA)
- Coordinating care with the assigned Clinical Team Manager, physician and other members of the home care team, informing them of significant changes in the patient’s condition and needs.
- Manage and coordinate the plan of care in a professional manner.
- Complete timely, accurate, and compliant clinical documentation.
REQUIRED EXPERIENCE:
- Minimum of Associate degree (or higher) in Nursing
- Current CA RN License (Required)
- 2 years of RN experience in Home Health
- Current CPR/BLS certification
- Ability to chart with Electronic Medical Record Software
