JOB DESCRIPTION: Manage and supervise all activities of the home/apartment units, including staff coverage, payroll, and implementation of ISP’s (Individual Support Plan). Medicaid billing and implementation of behavior and individual programs. Handle crisis management and the purchasing of household/food/clothing supplies in compliance with program reporting requirements. Responsible for Daily Data Collection sheets which supports the active treatment plans to include behavior plans, Medicaid services documentation and health and safety forms. Ensure compliance of all funding sources, regulations and agency policies. Provide quality service and coordination among shifts.

ESSENTIAL FUNCTIONS AND RESPONIBILITIES:

STAFF SCHEDULE:
- Preparation of work schedule for staff on all shifts assigned to home/apartment ensuring coverage per Medicaid ICF-MR/Waiver services.
- Prepare bi-weekly staff payroll and performance management related to staff attendance and performance.
ISP/IPP/BSP DELIVERY:
-
Attend ISP meetings, case conference, medication review, psychiatric consultation and program review meetings.

Participation as an active member of the Interdisciplinary Team.
-
Ensure all program implementation is documented appropriately and accurately.
-
Responsible for ensuring and overseeing ISP/Behavior Plan implementation, staff training/modeling, data collection, and activity schedules.
-
Responsible for assisting with and providing crisis preventative intervention services under the training and direction of the Psychologist.
-
Responsible for documentation of crisis intervention services per requirements and accurate documentation of all targeted behaviors in the homes/apartments.
DOCUMENTATION OVERSIGHT:
-
Review and ensure accuracy of monthly active treatment/behavior date collection forms and Medicaid documentation.
-
Ensure documentation of all program reporting requirement, i.e. incident reports, Medicaid, crisis prevention, logs, alarms, fire drills, water temperature etc.

Ensure homes/apartments have all necessary documentation forms-incident reports (Agency/DDS), active treatment behavior data collection, Medicaid data collection forms, health and safety forms, house log, visitors log, transportation logs, emergency evacuation/fire drills, household supplies/grocery lists, time sheets, general requisitions, maintenance/repair logs, etc.
DIETRY OVERSIGHT:

Responsible for ensuring menus are in the home/apartment, staff are trained on menus and special diets.

Ensuring food shopping incorporates menu and client preferences.
RESIDENTIAL OVERSIGHT:
-
Responsible for ensuring all locations assigned has sufficient household/food/clothing supplies.

Ensuring the overall operation of all locations assigned to include general environment, utilities, health and safety.
-
Responsible for all shopping related to overall operation of assigned locations.
-
Ensure the health & safety of the environment.
RECREATION:
-
Responsible for planning and preparing recreation calendar and home visit schedules.
-
Coordinate weekend services/activities and staffing schedules.
MEDICAL OVERSIGHT:
-
Responsible for assisting in medical emergencies to include coordination of transportation/staffing for medical appointments and accompanying people supported on medical appointments as necessary.

Ensure open communication with nursing and medical services staff to ensure people supported safety and overall well-being.
REGULATORY COMPLIANCE:
-
Responsible for compliance of staff as it relates to agency regulations, policies and procedures and other applicable regulations-Medicaid waiver, DDS policies and Human Care Agreement regulations including protection of people supported rights and safety.
TRANSPORTATION:
-
Ensure safe operation of the vehicle assigned to the program.
-
Follow transportation and vehicle maintenance policies of agency.
-
Ensuring vehicle has sufficient gas, is safe and maintains vehicle documentation per agency policy.
-
Ensuring all staff are trained and approved by the management.

FINANCIAL RESPONSIBILITY:
-
Submission of all grocery, household, clothing and other receipts to QIDP, Finance Department.
-
Maintaining and accounting for petty cash.
-
Responsible for ensuring timely and accurate submission of client funds requisitions to QIDP and submission of necessary receipts to the agency Finance Department.

BILLING:
-
Bi-monthly preparation of Medical billing per agency procedures and timely submission of documents to Finance Department.
-
Ensuring all prior authorizations are current for clients and ensuring the tracking of services.
-
Perform other duties as assigned.
MANAGEMENT RESPONSIBILTIES:

Supervise and schedule DSP’s

Train new staff in IPP, new staff orientation overview, agency policies and procedures as needed

Responsible for staff payroll

Responsible for compliance of staff as it relates to agency’s regulations, policies and procedures

Responsible for ensuring supplies are available in all locations at all times including weekends
QUALIFICATIONS: (Required to perform essential functions)

EDUCATION/EXPERIENCE:

High school diploma
-
Bachelor’s degree (B.A.) preferred in Human Service or related field
- Six years supervisory experience working with Developmentally Disabled/Emotional Disturbed and/or multiple handicap population
LANGUAGE SKILLS:

Ability to effectively communicate orally and in written communication form to internal/external agencies, vendors/clients, staff and management, and maintain professional relationships.

Ability to read and comprehend simple instructions, business correspondence and memorandums.
