Responsibilities
- Deliver direct patient care by assisting with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts.
- Monitor and document vital signs, medication administration, and changes in patient condition accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, ensuring compliance with HIPAA and home healthcare regulations.
- Observe patients closely for signs of health deterioration or behavioral changes, including those related to dementia, Alzheimer's disease, or other cognitive impairments.
- Assist with specialized care needs such as catheter care, wound dressing, end-of-life support, and managing memory or hospice care requirements.
- Support patients with mobility and transfer techniques while maintaining safety protocols for heavy lifting and patient handling.
- Maintain a clean and safe environment by performing light housekeeping tasks like laundry, meal preparation, and general cleaning to promote overall well-being.
- Communicate effectively with clients, families, and healthcare team members to ensure seamless delivery of personalized care plans.
Skills
- Proven experience working with geriatric patients, individuals with developmental disabilities or disabilities requiring specialized attention such as dementia or Alzheimer's care.
- Strong knowledge of medical terminology, medication administration procedures, vital signs monitoring, and basic life support (BLS) including CPR.
- Familiarity with home health agency operations, home & community care standards, and hospice or end-of-life care protocols.
- Ability to operate EMR/EHR systems efficiently for medical documentation and record keeping.
- Excellent patient personal care assistance skills combined with compassionate client communication.
- Physical stamina for activities involving heavy lifting, patient transfers using methods like Hoyer lifts or patient transfer techniques.
- Understanding of HIPAA regulations to maintain patient confidentiality at all times.
- Experience working with individuals with disabilities or in long-term care environments is highly valued.
- Ability to adapt quickly to different settings such as assisted living facilities or home healthcare environments while adhering to safety standards.
