Mercy Circle is a vibrant continuing care retirement community sponsored by the Sisters of Mercy, located on Chicago’s south side. We provide a full continuum of care—independent living, assisted living, memory care, and skilled nursing—to adults 62 and older in a mission-driven, person-centered environment.
We are seeking a detail-oriented, quality-focused Registered Nurse to join our leadership team as the MDS Coordinator (RN). This is a full-time, benefit-eligible role responsible for overseeing the MDS process and supporting clinical operations, while also serving in a leadership capacity with manager and on-call responsibilities.
What You’ll Do
As the MDS Coordinator (RN), you will combine clinical expertise, regulatory knowledge, and leadership skills to ensure accurate assessments, optimal reimbursement, and high-quality resident care.
MDS & Care Coordination
- Coordinate and complete the Minimum Data Set (MDS) process in accordance with federal and state regulations
- Ensure accurate and timely completion of assessments, care area assessments (CAAs), and care plans
- Collaborate with interdisciplinary teams to develop and update individualized resident care plans
- Monitor clinical documentation to support MDS accuracy and reimbursement optimization
- Track and manage MDS scheduling and submission timelines
Quality, Compliance & Reimbursement
- Ensure compliance with CMS guidelines, PDPM requirements, and Illinois state regulations
- Monitor quality measures and clinical outcomes, implementing improvement initiatives as needed
- Conduct regular chart audits and provide feedback to clinical staff
- Support and participate in the annual state survey process and audits
- Serve as a resource for clinical documentation and coding accuracy
Leadership & Operations
- Serve in a leadership role supporting day-to-day clinical operations within Skilled Nursing
- Supervise, coach, and support nursing staff as needed to ensure high-quality resident care
- Foster a collaborative, resident-centered culture that reflects Mercy Circle’s mission and values
- Assist with staffing coordination and operational decision-making in collaboration with nursing leadership
Staffing, Development & Support
- Provide education to nurses and interdisciplinary team members on MDS processes and documentation standards
- Support onboarding and ongoing training of clinical staff
- Participate in performance coaching and staff development initiatives
On-Call & Manager Responsibilities
- Participate in the Clinical Manager On-Call (MOD) rotation, providing leadership support across the community
- Step in to provide direct clinical support, including working on the floor, when staffing coverage cannot be secured to ensure continuity of care and resident safety
- Maintain flexibility to work evening, night, weekend, and holiday hours to support operations and engage staff across all shifts
Qualifications
- Current Registered Nurse (RN) license in Illinois
- Graduate of an accredited School of Nursing
- 3–5+ years of nursing experience in a skilled nursing or long-term care setting
- Previous MDS coordination experience required
- Strong knowledge of CMS regulations, PDPM, and MDS processes
- Experience with quality measures and regulatory compliance
- Leadership or supervisory experience preferred
- Strong attention to detail, organization, and analytical skills
- Excellent communication and interpersonal skills
- CPR certification and computer proficiency
