Position: Social Worker
Facility: Nursing and Rehab
Location: Nassau County, NY
Schedule: Full-Time | Monday–Friday | 9:00 AM – 5:00 PM
Salary: $68,000 – $70,000 per year
Nursing and Rehab is seeking a compassionate and dedicated Social Worker to join our team. In this rewarding role, you will help enhance the quality of life of our residents by addressing their emotional, social, and psychological needs while providing guidance and support to residents and their families throughout their care journey.
Conduct psychosocial assessments and interviews with residents and families to develop individualized care plans
Collaborate with residents, families, and interdisciplinary team members to promote independence, dignity, and overall well-being
Assist with discharge planning and coordinate resources to support successful transitions of care
Provide counseling, support, and advocacy for residents experiencing social or emotional challenges
Connect residents and families with appropriate community and financial resources
Maintain accurate documentation in accordance with facility policies and regulatory requirements
Participate in care plan meetings and interdisciplinary team discussions
Foster positive communication between residents, families, and staff members
Minimum of 2 years of Social Worker experience in a Skilled Nursing Facility (SNF) required
Bachelor's Degree in Social Work or related field required
Strong communication, interpersonal, and organizational skills
Compassionate, resident-centered approach to care
Experience with Sigma EMR preferred
Ability to work effectively within a multidisciplinary team
