Duties
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and detailed health histories to develop individualized care plans.
- Administer medications via injections, IV infusions, and other routes, ensuring adherence to aseptic techniques and safety protocols.
- Perform specialized procedures such as wound suturing, ostomy care, catheterization, and tube feeding with precision and compassion.
- Manage complex medical conditions including acute pain management, infectious disease care, hospice & palliative medicine, and trauma medicine.
- Educate patients and their families on disease management, medication administration, health coaching, and post-discharge care instructions.
- Document all patient interactions accurately using EMR (Electronic Medical Records) systems like Epic or Cerner while maintaining strict compliance with HIPAA regulations.
- Coordinate with interdisciplinary teams for discharge planning, case management, and long-term care needs across various settings such as assisted living facilities, nursing homes, and outpatient clinics.
Qualifications
- Current RN licensure in good standing with the ability to practice in the state of employment.
- Proven experience in home health nursing or related fields such as hospital medicine, ICU experience, or medical-surgical units.
- Familiarity with EMR/EHR systems including Epic, Cerner, or Athenahealth for accurate documentation.
- Strong knowledge of medical terminology, physiology, ICD coding (ICD-10/ICD-9), and diagnostic evaluation techniques.
- Demonstrated skills in IV insertion, phlebotomy, specimen collection/processing, and aseptic technique.
- Experience working with diverse patient populations including pediatrics, geriatrics (including dementia & Alzheimer's care), individuals with disabilities or developmental disabilities.
- Ability to perform physical assessments and vital signs monitoring accurately; proficiency in airway management and ventilator support preferred.
- Critical care experience such as ICU or trauma center background is highly desirable.
- Excellent communication skills for triage calls, patient education, and interdisciplinary collaboration.
