Responsibilities
The essential function of the NFMH case manager is to work in partnership with Nursing Facilities for Mental Health (NFMH), and other community partners in providing specialized services to the consumers in the facilities. The CM should empower the individual to make an informed choice on transitioning into the community. The CM should work under a strengths-based approach and the majority of service is provided in the NFMH or community setting. Case managers must be able to transport clients, have a reliable vehicle, a valid Kansas driver’s license and necessary insurance coverage, must be able to pass a KBI check, and must not be listed on any registries. Some availability after working hours is required. Each case manager is responsible for an assigned caseload and will provide back-up to another case manager’s caseload.
Job Requirements
Education and Licensure:
- Bachelor’s degree in behavioral health discipline (Social/Human service field with one year of experience substituting for one year of education.
- Must successfully pass Kansas Bureau of Investigation (KBI), Central Registry, and all required background checks.
- A valid Kansas driver’s license and access to reliable personal transportation and necessary insurance is required.
Experience:
- Minimum of one (1) years providing services in behavioral health.
- Familiarity with trauma-informed, evidence-based, and culturally responsive care.
Salary Range
$23.00 per hour
(Will consider experience/education for higher starting wage)
Hours
Mon – Fri, Hours may vary between 7am – 7pm to meet program and participant needs.
