Responsibilities
- Provide one-on-one physical therapy services to children and adults in their homes.
- Conduct comprehensive evaluations to assess mobility, strength, balance, coordination, and functional abilities.
- Develop, implement, and update individualized, goal-oriented treatment plans in accordance with physician orders and agency policies.
- Deliver evidence-based, patient-centered care using therapeutic exercises, manual therapy techniques, and assistive devices.
- Educate patients and caregivers on home exercise programs, safe mobility techniques, and fall prevention.
- Collaborate with families, caregivers, physicians, and interdisciplinary team members to ensure continuity of care.
- Monitor patient progress and modify treatment plans as appropriate to achieve the best possible outcomes.
- Maintain accurate, timely, and compliant clinical documentation.
- Participate in interdisciplinary meetings and care coordination as needed.
- Adhere to all professional, ethical, and organizational standards of practice.
Qualifications
- Current New York State Physical Therapist license.
- Experience working with pediatric and/or adult populations.
- Experience with individuals with Intellectual and Developmental Disabilities (ID/DD) is preferred.
- Home care and/or community-based experience is a plus.
- Strong clinical assessment and communication skills.
Compensation
- $65–$70 per 30-minute treatment session- 2 PATIENTS MAKE $120-$130 HR
- Opportunity to complete two sessions per hour, maximizing your earning potential.
Work Schedule
- Flexible daytime schedule based on your availability.
- Cases assigned based on your preferred service area and ZIP code.
- One-on-one treatment sessions conducted in patients' homes.

- Per diem opportunities are available.
- We also offer supervision-only positions for qualified therapists who are interested in providing supervisory services.
