POSITION SUMMARY
The Patient Care Liaison acts as a bridge between patients and the healthcare team, facilitating communication, addressing concerns, and providing guidance throughout the patient's stay.
ESSENTIAL FUNCTIONS
Assisting Patients and Visitors
- Provides service by greeting and actively helping patients and visitors who need assistance with wayfinding.
- Transports patients by wheelchair as needed.
- Deals calmly and courteously with upset or challenging visitors.
Supplies & Equipment
Adapting to the Environment
- Adapts and responds quickly to changing priorities in a high-volume area.
- Organizes and makes changes to the physical environment to ensure patient needs are met.
- Cross-trains in multiple areas and departments of the facility to support team needs.
Cleaning & Organization
- Cleans patient rooms, wipes surfaces, and removes waste in accordance with infection control protocols.
- Cleans and sanitizes patient care items and devices, keeping them ready for use.
- Keeps work areas, patient care areas, and transport vehicles organized.
- Prepares and organizes patient charts.
Communication
- Answers Vocera.
- Communicates and reports patient data and information to the RN to ensure appropriate and timely care.
- Utilizes basic computer software, hardware, and communication devices to perform tasks.
- Effectively communicates verbally and in writing.
Safety Monitoring
- Participates in monitoring, identifying, and reporting safety-related issues.
Quality and Compliance
- Adheres to all facility policies, regulatory requirements, and confidentiality standards.
- Participates in quality improvement initiatives.
- May be required to participate in internal audits as assigned.
- Reports concerns to leadership.
- Participates in continuing education and annual competencies.

EDUCATION & EXPERIENCE
- High school diploma or GED required.
- 2 years of customer service experience preferred.
