Case Manager / Aftercare Coordinator – Residential Treatment Center (RTC)

Position Summary
We are seeking a dependable, organized, and compassionate Case Manager / Aftercare Coordinator to coordinate services for children and adolescents residing in our Residential Treatment Center.
The Case Manager will serve as a primary point of coordination between each resident, the resident’s family or managing conservator, DFPS or the applicable Single Source Continuum Contractor (SSCC), therapists, LSOTP staff, medical providers, schools, attorneys, court representatives, residential staff, and other members of the treatment team.
This is a full-time, on-site position with significant responsibility for maintaining complete and audit-ready resident files, coordinating individualized service plans, scheduling appointments and treatment-team meetings, monitoring deadlines and resident progress, supporting admissions and discharge planning, and helping ensure that each resident receives the services identified in their individualized service plan.
The Case Manager will also take an active role in aftercare planning and coordination, helping ensure residents have appropriate services, appointments, resources, and supports in place as they transition from the facility.
Essential Duties and ResponsibilitiesCase Management and Service Planning
- Manage an assigned caseload of residents and maintain regular contact with each resident.
- Review placement documentation, assessments, medical information, education records, court orders, safety plans, and other relevant records upon admission.
- Coordinate the completion and implementation of preliminary, initial, updated, and discharge service plans.
- Work with therapists, LSOTP staff, caregivers, nurses, administrators, families, caseworkers, medical providers, and other professionals to identify each resident’s physical, emotional, behavioral, educational, medical, dental, and developmental needs.
- Monitor each resident’s progress toward service-plan and treatment goals and recommend updates when needs or circumstances change.
- Ensure staff working with the resident understand applicable service-plan goals, supervision requirements, safety plans, restrictions, appointments, and follow-up responsibilities.
- Identify unmet needs and make timely referrals to appropriate professional and community resources.
Meetings and Treatment-Team Coordination
- Schedule and coordinate initial service-plan meetings, service-plan reviews, treatment-team meetings, discharge meetings, staffing meetings, and other required conferences.
- Prepare and distribute required meeting notices within applicable timeframes.
- Coordinate participation from families, managing conservators, DFPS/SSCC representatives, therapists, LSOTP staff, medical professionals, school personnel, attorneys, court-appointed representatives, caregivers, and other required participants.
- Prepare agendas, gather supporting documentation, record attendance, take meeting notes, obtain required signatures, and distribute completed plans or meeting summaries.
- Follow up with individuals who do not respond to meeting notices and document attempts to obtain participation.
- Participate in multidisciplinary treatment-team meetings and provide relevant information regarding each resident’s progress, needs, appointments, services, and discharge planning.
Resident Records and Documentation
- Maintain organized, accurate, current, and audit-ready physical and electronic records for residents on the assigned caseload.
- Complete case notes and document significant contacts, appointments, meetings, incidents, behaviors, services, progress, concerns, and follow-up actions.
- Track missing, expired, or incomplete documentation and obtain corrections promptly.
- Maintain admission documents, placement agreements, service plans, medical and dental records, immunization records, education records, court documents, consents, medication information, incident reports, family-contact records, and discharge documentation.
- Review files regularly for compliance with Texas Minimum Standards, DFPS/SSCC contractual requirements, facility policies, and applicable T3C requirements.
- Prepare files and requested documentation for licensing inspections, contract monitoring, internal audits, investigations, and quality-assurance reviews.
- Protect confidential information and release records only to properly authorized individuals.
Medical, Dental and Behavioral Health Coordination
- Schedule and coordinate medical, dental, vision, psychiatric, psychological, therapy, LSOTP, and other required appointments.
- Coordinate transportation and, when assigned, safely transport residents to appointments, court hearings, school meetings, family visits, and other approved activities.
- Ensure required consent documents, identification, medical information, medication lists, and appointment paperwork accompany the resident when necessary.
- Obtain appointment summaries, recommendations, prescriptions, follow-up instructions, and supporting documentation following visits.
- Communicate relevant recommendations to authorized staff and ensure required follow-up appointments or services are scheduled.
- Work closely with nursing and medication-trained staff to maintain accurate health-related documentation without independently performing duties that require a medical license.
- Assist in coordinating residents’ medical, dental, behavioral health, educational, and other service needs.
- Maintain familiarity with available STAR Health services and assist the treatment team with coordinating appropriate services for eligible residents.
Aftercare Services and Discharge Coordination
- Begin aftercare and transition planning in coordination with the treatment team prior to discharge.
- Develop and coordinate individualized aftercare plans based on each resident’s identified needs, treatment progress, discharge destination, and available community resources.
- Work with residents, families, caregivers, managing conservators, DFPS/SSCC caseworkers, therapists, LSOTP providers, schools, medical providers, and community organizations to establish appropriate post-discharge services.
- Assist with arranging follow-up therapy, psychiatry, medical care, medication management, educational services, community programs, and other recommended services before discharge when applicable.
- Identify potential barriers to successful discharge and work with the treatment team to address concerns before transition.
- Ensure discharge summaries, medication information, appointment information, education records, personal documents, belongings, and other required materials are appropriately prepared and transferred.
- Provide families and caregivers with information regarding recommended services, community resources, appointments, and follow-up expectations.
- Maintain appropriate aftercare communication and follow-up with authorized parties to support continuity of care after discharge.
- Document aftercare contacts, referrals, follow-up efforts, outcomes, barriers, and concerns.
- Notify the Program Director, Treatment Director, or other appropriate leadership of significant aftercare concerns requiring additional intervention.
- Maintain accurate documentation of discharges, transfers, placement changes, interruptions in service, and aftercare referrals.
Communication and Advocacy
- Maintain professional communication with residents, families, managing conservators, DFPS/SSCC caseworkers, therapists, LSOTP staff, schools, courts, attorneys, medical providers, residential staff, and community partners.
- Provide timely updates regarding resident progress, appointments, significant incidents, service needs, placement concerns, and discharge planning.
- Advocate for services and resources that support each resident’s safety, stability, permanency, education, health, and emotional well-being.
- Assist residents in understanding and participating in their individualized service plans in a developmentally appropriate manner.
- Help identify and resolve barriers to service delivery and elevate unresolved concerns to the Program Director, Treatment Director, or Administrator.
Admissions and Transitions
- Assist with the admission process and verify that required placement, consent, medical, educational, and supporting documentation has been received.
- Assist with orienting newly admitted residents to the facility, their rights, available services, schedules, expectations, and complaint procedures.
- Coordinate transition and discharge planning with the treatment team, family, caseworker, school, medical providers, and receiving placement.
- Support continuity of services during placement changes, transfers, and discharges.
Compliance and Team Responsibilities
- Follow Texas Minimum Standards for General Residential Operations, DFPS/SSCC requirements, applicable T3C standards, facility policies, and reporting requirements.
- Immediately report suspected abuse, neglect, exploitation, policy violations, or threats to resident safety through required reporting channels.
- Participate in staff meetings, case consultations, training, quality-assurance activities, and corrective-action planning.
- Maintain required certifications and complete all required pre-service and annual training.
- Support the facility’s Evidence-Informed Treatment Model and remain current with required program practices and training.
- Assist residential staff during urgent situations when needed while maintaining the primary responsibilities of the case-management role.
- Perform other reasonable case-management and program duties assigned by the Administrator or Program Director.
Required Qualifications
- Bachelor’s degree in social work, psychology, counseling, sociology, criminal justice, child development, human services, or a closely related field.
- At least one year of experience in child welfare, foster care, behavioral health, residential treatment, case management, juvenile services, or a related setting.
- Strong organizational, documentation, communication, and time-management skills.
- Ability to manage multiple deadlines, appointments, meetings, service plans, and resident records simultaneously.
- Ability to communicate professionally with residents, families, caseworkers, medical professionals, therapists, attorneys, school personnel, and regulatory representatives.
- Working knowledge of trauma-informed care and the needs of children and adolescents who have experienced abuse, neglect, family separation, behavioral challenges, or mental-health concerns.
- Proficiency with Microsoft Office, email, electronic recordkeeping systems, and virtual-meeting platforms.
- Valid Texas driver’s license, reliable transportation, acceptable driving record, and ability to meet the facility’s vehicle-insurance requirements.
- Ability to pass all required criminal-history, central-registry, reference, employment-history, and other required background checks.
- Ability to maintain confidentiality and exercise sound professional judgment.
Preferred Qualifications
- Two or more years of experience in a Texas RTC, GRO, foster-care program, behavioral-health program, or similar residential childcare environment.
- Previous experience working with DFPS, Community-Based Care contractors, SSCCs, court systems, or Texas Child-Centered Care requirements.
- Experience developing individualized service plans, preparing progress reports, coordinating treatment-team meetings, and maintaining audit-ready resident files.
- Experience coordinating discharge and aftercare services for children or adolescents.
- Knowledge of STAR Health and services available to children in foster care.
- Current CPR and First Aid certification.
- Training in trauma-informed care, crisis intervention, behavior management, psychotropic medication, medical consent, transportation safety, and recognizing and reporting abuse and neglect.

The successful candidate will understand that effective case management does not end at discharge and will take an active role in helping residents and their support systems establish the services and resources necessary for a successful transition from residential care.
Important Role Boundary
The Case Manager / Aftercare Coordinator coordinates, monitors, and facilitates services but does not replace the resident’s therapist, LSOTP, psychiatrist, nurse, or other licensed treatment provider. Clinical counseling, diagnoses, medical decisions, specialized treatment, and other licensed services may only be performed by individuals with the appropriate professional license and authorization.
Pay: Up to $60,000.00 per year
