Admit/Flex Nurse (RN)

We are seeking a compassionate and adaptable Admit/Flex Nurse (RN) to join our dedicated interdisciplinary team serving patients across Maui County.
This role is ideal for an experienced RN who thrives in a flexible environment, enjoys building meaningful patient relationships, and is passionate about providing exceptional palliative and hospice care in homes, hospitals, facilities, and community settings.
What You’ll Do
As aAdmit/Flex Nurse, you will:
- Conduct palliative care and hospice admissions and comprehensive nursing assessments
- Coordinate smooth transitions from hospital to home, facility, or Hospice Hale
- Provide compassionate direct patient care and symptom management
- Educate and support patients, caregivers, and families
- Collaborate closely with physicians, social workers, chaplains, aides, and the interdisciplinary team
- Communicate with referral sources and community partners
- Complete accurate clinical documentation and care planning
- Help foster Hospice Maui’s reputation for excellence and compassionate care throughout the community
Qualifications
- Current Hawaii RN license in good standing
- Current CPR certification
- Valid driver’s license and reliable transportation
- Minimum two years of nursing experience preferred
- Hospice, palliative care, home health, or hospital experience preferred
- Strong communication, critical thinking, and organizational skills
- Comfortable working independently while collaborating with a supportive team
- Commitment to hospice and palliative care philosophy

Schedule
- Flexible nursing schedule
- Generally Mon-Fri with occasional weekends

- Hospitals
- Skilled nursing facilities
- Hospice Hale inpatient unit

Pay: $45.07 - $47.57 per hour
