This is a paraprofessional position designed to assist case managers with routine work, including; intakes, updates, liabilities, waiver eligibilities, and other tasks as assigned. Strong organizational, creative and interpersonal skills required. Must be flexible and solutions oriented.
Position Requirements
To perform the Social Services Aide 3 job successfully, an individual must be able to perform each essential responsibility satisfactorily. These requirements are representative, but not all-inclusive, of the knowledge, skill, and ability required.

Education and Experience

One year as a County Social Services Aide 2;
OR
-
Graduation from high school, or its equivalency, and three years of experience in public contact work in a human services agency which included one year of experience in interviewing and obtaining information;
OR

An associate's degree in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences, and one year of experience in a human services agency;
OR
-
Any equivalent combination of experience and training.
Physical Demands
While performing the responsibilities of the Social Services Aide 3, the employee is required to talk and hear. The employee is often required to sit and use their hands and fingers, to handle or feel and to manipulate keys on a keyboard. The employee may be required to stand, walk, reach with arms and hands, climb or balance, stoop, kneel, crouch or crawl and, enter and exit a vehicle. The work may require constant mobility. The employee may need to work with arms above shoulder height, bend at the waist or work bent at the waist, lift or carry items or occasionally work with an uncommon level of noise. Vision abilities required by this job include close vision.
Work Environment
Normal office environment. The noise level in the work environment is usually quiet to moderate. Position may require travel throughout Carbon, Monroe and Pike counties in addition to periodic travel to other locations in Pennsylvania. An agency vehicle is usually available during work hours.
Position Duties:
Duties: This is a paraprofessional position designed to assist social service staff. At times, this paraprofessional may be requested to assist case managers with routine work, including; intakes, updates, liabilities, waiver eligibilities, and other tasks as assigned. This position may be required to assist the support staff with phone, liability and office coverage.

Prepare and distribute materials for Administrative Entity Meetings, including meeting agendas and supportive documents, attend meetings, prepare minutes, distribution of minutes.

Assisting with the AE duties that include fiscal year renewal, Quality Assessment and Improvement, Fair Hearing and Appeals, and preparation and distribution of correspondence.

Assisting with completion of MA applications for individuals at intake, and also when renewal is due for current consumers.

Confirm intake appointments, act as a backup on conducting intakes as needed.

Assisting in obtaining information to complete transfers of individuals to other counties.

Tracking and maintaining spreadsheets for Autism only physicals and re-certifications.

Maintain electronic records for intakes and waiver files.

Assist the Administrative Entity with preparing documentation for waiver recertification

Assist the Administrative Entity with tracking ISP Signature Page submissions

Develop and distribute a monthly calendar inclusive of natural supports and opportunities for community inclusion

Act as BP Administrator for HCSIS

Initiate bed searches for individuals requiring residential placement. Develop and maintain spreadsheets to track progress.

Prepare and distribute materials for Carbon-Monroe-Pike's Regional Collaborative, including meeting agendas, supporting documents, attend Regional Collaborative meetings, prepare minutes, and distribute minutes
Additional Duties:
