Responsibilities:
As a Patient Care Liaison, you will:

Serve as the primary point of contact for new client referrals and service requests.

Monitor intake systems and accept new cases promptly, ensuring timely assignment to care coordination teams.

Verify insurance eligibility, authorizations, and client information to support uninterrupted care.

Process referrals and authorizations accurately within established deadlines.

Prepare daily recovery reports and communicate care updates to clients.

Coordinate onboarding activities, including welcome calls and verification of demographic, insurance, and emergency contact information.

Assign caregivers to new cases and confirm schedules, service details, and client needs.

Facilitate smooth transitions between intake, scheduling, and ongoing care management teams.

Provide exceptional customer service by responding to client, caregiver, and partner inquiries.

Support scheduling activities and maintain accurate documentation in the electronic medical record (EMR) system.

Collaborate with internal departments, including coordination, billing, EVV, and payer relations, to prevent service disruptions.

Assist with eligibility audits, authorization renewals, and continuity of care planning.

Track admission and referral outcomes and identify trends to support operational improvements.

Conduct client satisfaction and follow-up calls to ensure a positive care experience.

Provide administrative and operational support to Patient Care Coordinators and Client Service Managers as needed.
