We are seeking an organized, personable, and highly motivated Patient Care Coordinator to help create an excellent experience for patients. This comprehensive role includes patient scheduling, reviewing appointment history and calendar flow, managing phone messages and shared task folders, processing and tracking faxes and documents, supporting chronic care management administration, assisting with insurance and billing follow-up, and promoting our Pathway Plan and patient education classes.
The ideal candidate has a high level of competency with computers, electronic health record systems, patient portals, digital scheduling tools, document management, and cloud-based phone systems. This person must be comfortable learning new software quickly, navigating multiple systems at once, troubleshooting basic technology issues, accurately documenting patient communication, and using shared electronic work queues to organize and complete tasks. Strong attention to detail, professional communication, and the ability to recognize when a concern should be escalated to a nurse or provider are essential.

Pay: $18.00 - $25.00 per hour
