Plans, organizes, and implements social work services for patients and families. Coordinates discharge planning services.

Primary Duties
Resident Assessment and Care Planning

Complete psychosocial assessments upon admission and as required thereafter.

Participate in comprehensive resident assessments, including Minimum Data Set (MDS) processes as assigned.

Identify psychosocial, emotional, behavioral, and environmental needs of residents.

Develop, implement, and monitor individualized social service interventions.

Document resident progress and interventions in accordance with regulatory requirements.

Coordination and facilitation of ancillary services for optometry, podiatry, dental. Coordinating outpatient referrals and appointments.
Resident and Family Support

Provide counseling, support, and crisis intervention to residents and families.

Assist residents in adjusting to placement in the facility.

Facilitate family conferences as needed.

Educate residents and families regarding available resources and services.
-
Address concerns, grievances, and complaints in accordance with facility policy.

Promote resident rights and advocate for resident preferences and choices.
Discharge Planning

Coordinate discharge-planning beginning at admission.

Assess resident discharge needs and barriers.

Collaborate with physicians, nursing staff, therapy staff, residents, and families regarding discharge goals.
