Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts when necessary
- Monitor patients’ vital signs and observe changes in health status to report promptly to healthcare professionals
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding if required
- Administer medications following care plans and ensure compliance with HIPAA regulations to protect patient privacy
- Maintain a clean and safe environment through regular cleaning, laundry, and organization of the home
- Support clients with memory care needs, including dementia or Alzheimer’s care, utilizing specialized techniques for safety and engagement
- Document care activities accurately, update care plans regularly, and communicate effectively with healthcare teams and family members
- Provide emotional support and companionship to promote mental well-being and social engagement
- Use proper techniques for heavy lifting or assistive device operation such as Hoyer lifts or resident lifts during transfers
- Participate in ongoing training on patient observation, CPR/First Aid certification renewal, and compliance with healthcare standards
Experience
- Prior experience in home health care, assisted living facilities, nursing homes, or group homes is highly preferred
- Demonstrated knowledge of patient monitoring, medication administration, and vital signs measurement
- Experience working with individuals with developmental disabilities, autism spectrum disorder, dementia, or memory care needs is advantageous
- Familiarity with HIPAA regulations and maintaining confidentiality in healthcare settings is essential
- Hands-on experience with care plans development and implementation for long-term or hospice care patients
- Strong background in caregiving roles involving patient observation, direct support activities, and social work principles is desirable
- Ability to perform heavy lifting safely while adhering to proper techniques for resident transfers using assistive devices
