We are seeking a dedicated Assistant Director of Nursing (ADON) / RN Case Manager who is passionate about delivering A Higher Standard of Care and making a meaningful impact on the lives of patients throughout our community.
About the Role
As an ADON / RN Case Manager, you will support clinical operations while managing a caseload of skilled nursing clients in their homes. This hybrid role blends hands-on clinical care, case management, and leadership responsibilities. You will serve as a clinical resource for field staff, collaborate with interdisciplinary team members, and help uphold our standards of excellence within the nursing department.

Key Responsibilities
Clinical & Case Management
- Provide skilled, compassionate nursing care in accordance with physician orders and individualized patient plans.

Conduct in-home assessments and develop personalized care goals.

Serve as the primary Case Manager for assigned clients, ensuring high-quality, coordinated care.

Educate patients and families on disease processes, medications, and home care best practices.

Document all care accurately and efficiently using our EMR system.

Maintain clinical records that meet state, federal, and BrightStar Care standards.
Leadership & ADON Support
- Assist the Director of Nursing with clinical oversight and quality assurance initiatives.

Support supervision, coaching, and development of field staff.

Participate in chart reviews, competency evaluations, onboarding, and ongoing training.

Collaborate with leadership to ensure compliance with all regulatory requirements.

Provide support during on-call rotations as needed.
Qualifications
- Active Registered Nurse (RN) license in the state of Indiana.

Minimum of 1 year of professional nursing experience.

Home health and Medicare experience strongly preferred.

Experience in a leadership, charge nurse, or supervisory role is a plus.

Strong critical thinking, organization, and time-management skills.

Ability to work independently in the field and manage multiple priorities.

Reliable transportation, active driver’s license, and auto insurance.
Compensation & Benefits
Compensation
- Salaried position: $70,000 – $85,000 annually (based on experience).
