Responsibilities
- Perform comprehensive patient assessments, including vital signs, and physical examinations.
- Administer medications and injections accurately following physician orders while adhering to safety protocols and HIPAA regulations.
- Assist with complex medical procedures such as sterile dressing changes using aseptic techniques.
- Utilize EMR (Electronic Medical Records) system for accurate documentation of patient care activities.
- Support patients with neurological conditions such as Alzheimer's disease or dementia through memory care strategies and behavioral health support.
- Provide specialized care for patients with infectious diseases, acute pain management needs.
- Educate patients and their families on health coaching topics including medication management, nutrition, and chronic disease management.
- Maintain compliance with all healthcare regulations including HIPAA standards and NCQA (National Committee for Quality Assurance) guidelines.
- Ensure accurate medical documentation and detailed nursing notes.
- Participate in infection control practices and sterile processing procedures to uphold safety standards across all clinical activities.
Experience
Candidates should possess a valid LVN license with active certification to practice in their respective state. Prior experience is highly valued in the following areas:
- Long-term care facilities including nursing homes, assisted living communities, or sub-acute care units with a focus on geriatrics and dementia/dementia-related conditions like Alzheimer’s disease.
- Experience working with diverse populations such as children, adults in outpatient clinics, or home & community-based care environments.
- Proficiency in advanced clinical procedures such as phlebotomy, venipuncture, and specimen collection/processing. Additional desirable skills include experience administering injections; ability to perform physical examinations; understanding of physiology and anatomy; expertise in pain management strategies; and strong case management capabilities. Pay: $20.00 - $24.00 per hour
