GENERAL DEFINITION OF WORK
Performs intermediate professional work conducting comprehensive MnCHOICES assessments for individuals seeking home and community-based services; evaluating physical, mental, social, and environmental health status; determining eligibility for long-term services and supports; developing individualized plans of care; and coordinating community resources to promote client independence and least-restrictive living. Work is performed under the limited supervision of a Health and Human Services Supervisor.

ESSENTIAL DUTIES:

Conduct comprehensive online MNCHOICES Assessments for clients by evaluating their physical, mental, social and environmental health status.

Determine applicants' level of care and needs per state and federal criteria.

Analyze assessed data and assist applicants in identifying relevant goals along with specific supports that could enhance clients' abilities to meet goals and live in the least restrictive environment.

Maintain broad knowledge base related to payer sources and community-based services.

Comprehend and assist in explanation of programs and provide referrals to appropriate community resources.

Determine eligibility for home and community-based programs (AC, BI, CAC, CADI, DD, EW waivers, and PCA) based on individualized client needs and wants.

Coordinate the provision of long-term services and supports to ensure:
- 1) appropriateness,
- 2) quality,
- 3) cost effectiveness.
- Complete a plan of care for client.

Perform other related duties as required or assigned.
QUALIFICATION REQUIREMENTS

MINIMUM QUALIFICATIONS:

A bachelor’s degree from an accredited four-year college or university with a major in social work and at least one year of home and community-based experience or;

A Bachelor’s degree in nursing with current licensure as a registered nurse along with public health certification and at least one year of home and community-based experience or;
-
A Bachelor’s degree from an accredited four-year college or university with a major in a closely related field and at least one year of home and community-based experience or;
-
Current license as a registered nurse with at least two years of home and community-based experience.
- Also, valid Driver’s License and appropriate certification and licensure.
KNOWLEDGE, SKILLS, AND ABILITIES
Thorough knowledge of Minnesota's home and community-based waiver programs and eligibility criteria; thorough knowledge of the MnCHOICES assessment process and applicable state and federal guidelines; working knowledge of community resources, payer sources, and long-term services and supports available in the region; ability to conduct comprehensive functional assessments and translate findings into individualized care plans; ability to interpret and apply complex program eligibility requirements; ability to exercise sound judgment in evaluating client needs and determining appropriate services; ability to manage a caseload with accuracy and attention to documentation requirements; ability to communicate effectively and establish productive working relationships with clients, families, service providers, and interdisciplinary teams.

PHYSICAL REQUIREMENTS
This work requires the frequent exertion of up to 25 pounds of force; work regularly requires sitting, speaking or hearing, using hands to finger, handle or feel and repetitive motions, frequently requires lifting and occasionally requires standing, walking, climbing or balancing, stooping, kneeling, crouching or crawling, reaching with hands and arms, tasting or smelling and pushing or pulling; work requires close vision, distance vision, ability to adjust focus, depth perception, color perception, night vision and peripheral vision; vocal communication is required for expressing or exchanging ideas by means of the spoken word and conveying detailed or important instructions to others accurately, loudly or quickly; hearing is required to perceive information at normal spoken word levels and to receive detailed information through oral communications and/or to make fine distinctions in sound; work requires preparing and analyzing written or computer data, using of measuring devices, operating motor vehicles or equipment and observing general surroundings and activities; work occasionally requires exposure to fumes or airborne particles, exposure to outdoor weather conditions, wearing a self-contained breathing apparatus and exposure to bloodborne pathogens and may be required to wear specialized personal protective equipment; work is generally in a moderately noisy location (e.g. business office, light traffic).
