Foundations Health & Wholeness is adding a full-time (32+ hours per week) Coordinated Entry Navigator for our Northeast Region serving Oconto, Menominee, Marinette, Shawano and Florence counties. The Coordinated Entry Navigator is responsible for supporting the local coordinated entry (CE) system within the local homeless coalition to improve access to housing and services for people experiencing homelessness. The position is responsible for supporting coordinated entry implementation, facilitating case conferencing, monitoring compliance, ensuring staff training, maintaining required agreements and documentation, conducting outreach and marketing, and identifying opportunities for system improvement.
Duties of the position include:
- Serve as the local coordinated entry expert for the local homeless coalition overseeing it across the entire geographic area of the coalition
- Ensure consistent implementation of the Coordinated Entry Policies and Procedures Manual
- Provide consistent and accurate flow of information between the local homeless coalition and WI BOSCOC
- Facilitate coordinated entry case conferencing at least monthly promoting multi-agency and multi-sector approach to referral coordination
- Expand outreach efforts to reach key subpopulations ensuring information is broadly distributed to coalition members and stakeholders
- Assist in orientating and supporting participating agencies in coordinated entry implementation and requirements
- Document, monitor and provide information and training needed for reporting and grant compliance
- Participate in after-hours plan for accessing emergency services
- Travel throughout the full geographic area of the local homeless coalition to support coordinated entry
- Conduct or support outreach activities in a variety of settings
Ideal Candidate will have:
- Bachelor’s Degree in Social Work, Human Services, Public Administration, Counseling, Community Development or related field preferred or equivalent combination of education and experience
- Experience working with people experiencing homelessness, housing instability or related crisis
- Experience coordinating with multiple agencies, systems or community partners
- Strong organizations skills and ability to manage multiple responsibilities, deadlines and compliance requirements
- Excellent advocacy and communication skills
- Ability to maintain confidentiality and exercise sound judgement
- Possess and maintain valid driver’s license and insurance
- Experience working with and providing support services to vulnerable youth
- Ability to work well with many different levels/organizations to achieve a common goal
- Experience with data collection, measuring and meeting grant outcomes
- Knowledge and training in the areas of positive youth development, crisis intervention, trauma informed care, strength-based interventions, and conflict resolution
