PRIDE Support & Services is seeking a dedicated team member as a Community Support Home Provider, where you will play a vital role in providing compassionate, comprehensive care within your home. This dynamic position involves supporting individuals with diverse needs, including mental health conditions, disabilities, and substance use disorders, to foster independence and improve quality of life. You will collaborate with multidisciplinary teams to deliver personalized care plans, coordinate community resources, and ensure a safe, nurturing environment for residents. Duties
- Assist residents with daily living activities, including medication management, and mobility support
- Help develop and implement individualized care plans in collaboration with healthcare providers and families
- Coordinate community resources such as social services, outpatient programs, and support networks to enhance resident well-being
- Maintain accurate documentation in compliance with HIPAA regulations
- Engage in ongoing patient service initiatives that promote recovery, resilience, and community integration

Pay: $30,000.00 - $50,000.00 per year
