Must be a Registered Nurse or Licensed Practical Nurse
Duties:
- Provide direct patient wound care.
- Collaborate with healthcare team members to develop and implement patient care plans
- Monitor and assess patient conditions, including vital signs, lab results, and symptoms
- Educate patients and their families on wound management, treatment options, and preventive care
- Document patient information accurately and timely in electronic medical records
- Adhere to HIPAA regulations to ensure patient privacy and confidentiality
- Provide health coaching and support to patients for lifestyle modifications and self-care management
- Coordinate with other healthcare providers to ensure continuity of care for patients transitioning between different healthcare settings (e.g., hospital to home care)
- Provide specialized care in areas such as hospice, acute care, ICU, or pediatrics as required
Skills:
- Proficient in performing physical examinations and assessments
- Knowledgeable in wound care and aseptic technique for infection prevention
- Experience in providing acute care interventions and treatments
- Familiarity with home care practices and procedures
- Competent in administering medications and IV therapy
- Skilled in gastric lavage procedures when necessary
- Proficient in using electronic medical record systems for documentation purposes
