Key Responsibilities:
- Conduct comprehensive, 45-minute home health risk assessments for Medicare Advantage plan members
- Review and document medical history, current conditions, and medication usage
- Educate patients about their health status and medications
- Identify gaps in care, offering recommendations to address them
- Complete annual wellness checks and post-discharge follow-ups, as required
- No treatment planning or prescribing involved
Qualifications:
- Current Nurse Practitioner license in Ohio
- Active CPR (BLS) certification
- Board-certified by AANP or ANCC
- MSN from an accredited school of nursing
- At least 6 months of nurse practitioner experience preferred
