We’re hiring a Home Health Physical Therapist for an exciting travel assignment in Northern California!
✨ Position Details:
- Mileage reimbursement in addition to pay package
- Home Health setting
- 40 hours guaranteed
✅ Requirements:
- Active PT License required
- DPT required if licensed after January 2016
- BLS/CPR certification required
- Valid Driver’s License required
- Must be 21 years or older
- Home Health experience preferred (1 year ideal, willing to train)
Responsibilities:
- Perform patient evaluations and individualized treatment planning
- Provide therapy interventions and discharge planning
- Complete clinical documentation accurately and timely
- Supervise assistants as assigned
