Brookhaven Health Care Facility is looking to add a Physical Therapist Assistant (PTA) to its rehabilitation team. In this role, you'll work under the supervision of a Physical Therapist to provide therapy services that help residents improve mobility, strength, balance, and overall function. RESPONSIBILITIES:
- Provide skilled physical therapy treatments under the direction of a Physical Therapist
- Assist with implementing individualized treatment plans designed to improve mobility, strength, balance, and functional independence
- Monitor resident progress and communicate treatment updates to the supervising Physical Therapist
- Complete required documentation, treatment records, and care plan updates accurately and timely
- Educate residents, families, and staff on therapy techniques, exercises, mobility, and adaptive equipment
- Assist with discharge planning, home exercise programs, and equipment recommendations
- Collaborate with physicians, nursing staff, and interdisciplinary team members to support resident outcomes
- Participate in staff training, in-services, and quality improvement initiatives
- Maintain therapy equipment, supplies, and treatment areas
- Perform additional duties as assigned
REQUIREMENTS:
- Associate Degree from an accredited Physical Therapist Assistant program approved by the American Physical Therapy Association
- Valid New York State Physical Therapist Assistant License required
- Newly licensed Physical Therapist Assistants encouraged to apply
- Skilled nursing facility experience preferred
- Strong communication, documentation, and organizational skills
- Ability to work collaboratively within an interdisciplinary healthcare team
