What You’ll Do
- Provide comfort-focused personal care including ADLs, grooming, and mobility support
- Assist with comfort measures and medications as directed
- Observe and report changes in patient condition with care and accuracy
- Offer emotional support and steady presence to patients and families
- Collaborate closely with nurses, social workers, and the interdisciplinary team
- Help maintain a peaceful, clean, and safe environment during visits
- Follow individualized hospice care plans with empathy and consistency
Who We’re Hoping to Meet
- Minnesota-licensed CNA
- Experience in hospice, palliative care, or oncology
- Compassionate, dependable, and emotionally grounded
- Comfortable supporting patients and families at end of life
- Strong communicator who values teamwork and trust
- Valid driver’s license and reliable transportation
