About the Role
Are you an experienced senior care leader who loves building strong teams and ensuring residents live their best lives? We are looking for a dynamic, hands-on Resident Care Coordinator (RCC) to anchor the care operations across our Assisted Living and Memory Care neighborhoods.
As a vital member of our leadership team, you will bridge the gap between clinical oversight, family communication, and floor operations. This isn't just a desk job—it’s a role for a visible leader who is passionate about setting high standards of care, supporting frontline staff, and maintaining a vibrant, compliant community.
What You’ll Do (Key Responsibilities)
Working in close partnership with our Executive Director and Wellness Director/LVN, you will champion daily operations through:
- Team Leadership & Scheduling: Lead, mentor, and inspire our Caregivers (PCAs) and Med Techs. You will handle hiring, onboarding, and scheduling, ensuring optimal floor coverage and a supportive, reliable team environment.
- Care Planning & ADL Management: Develop, implement, and update tailored service plans that accurately reflect each resident’s unique needs, strengths, and daily routine.
- The Liaison for Families & Providers: Act as the primary point of contact for families, physicians, home health, and hospice partners to ensure seamless continuity of care.
- Compliance & Excellence: Maintain a sharp eye on safety and quality, ensuring absolute alignment with California Title 22 regulations for RCFEs and specialized memory care.
- Connected Leadership: Conduct daily rounds to check in with residents and monitor changes in condition. To stay deeply connected to our team and the nuances of our care, you will spend at least one shift per week working directly "on the floor."
- Interdisciplinary Collaboration: Partner with our Activities, Culinary, and Maintenance teams to deliver an exceptional, holistic lifestyle for our residents.
What We Are Looking For (Requirements)
- Proven supervisory experience in an Assisted Living or Memory Care setting.
- Deep familiarity with California Title 22 regulations for RCFEs.
- Strong organizational skills with a knack for scheduling, managing shift relief, and adjusting floor coverage on the fly.
- Exceptional communication skills—someone who can navigate tough family conversations with empathy and lead staff with clarity.
- A "lead from the front" mindset, willing to jump in on the floor alongside the team when needed.
Job Type: Full-time
Pay: $48,000.00 - $54,000.00 per year

- When a resident's cognitive or physical needs increase, families and POAs can become overwhelmed or resistant to adjustments in care levels. Describe a time you had a difficult conversation with a family member about their loved one's changing needs. How did you handle their concerns while ensuring the community's safety and operational standards were met?
- As an RCC, you are a leader on the floor. How do you train and hold your care team accountable to high clinical and safety standards? Describe a scenario where you noticed a caregiver or Med-Tech bypassing a critical protocol and how you handled the course correction.
- Managing a Memory Care neighborhood requires distinct strategies compared to general Assisted Living. How do you train floor staff to handle challenging, dementia-related behaviors (such as exit-seeking or combativeness) without escalating the resident's distress?
