Responsibilities
- Conduct initial client assessments and reassessments as required.
- Review, and update individualized Plans of Care.
- Supervise Personal Care Aides (PCAs), Certified Nursing Assistants (CNAs), and caregivers.
- Conduct supervisory visits in clients' homes.
- Be available by phone for questions and guidance.
- Maintain accurate and timely documentation.
- Sign off on weekly time and task sheets
