Overview
We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team. The ideal candidate will possess a strong background in pharmacy and demonstrate a commitment to providing exceptional patient care. This role involves collaborating with healthcare professionals, managing medication therapy, and ensuring the safe and effective use of pharmaceuticals in various settings, including acute care and long-term care (LTC) environments.
Duties
- Review and interpret physician orders to ensure safe and effective medication therapy.
- Provide patient assessments and counseling on medication use, side effects, and interactions.
- Manage inventory levels, including ordering, receiving, and storing pharmaceuticals.
- Administer immunizations and manage medication administration processes.
- Collaborate with healthcare teams to optimize patient care plans, including pain management strategies.
- Maintain compliance with HIPAA regulations and ensure confidentiality of patient information.
Skills
- Proficient in using pharmacy software for medication management and patient records.
- Ability to perform patient assessments effectively to tailor medication therapies.
- Familiarity with compounding medications according to established guidelines.
