Join a patient-focused healthcare team as an LPN/LVN, providing high-quality clinical care in a fast-paced primary care setting. You'll assess patients, administer treatments, perform screenings, support disease management, and help deliver exceptional patient experiences.
Key Responsibilities
- Administer medications, vaccines, and treatments.
- Perform phlebotomy, point-of-care testing, and routine patient assessments.
- Monitor vital signs and identify abnormal findings.
- Document patient histories and care plans in EMR systems.
- Conduct medication reviews and patient education.
- Support mental health screenings and wellness assessments.
- Coordinate patient care and connect patients with community resources.
- Maintain a clean, organized, and well-stocked clinic environment.
Qualifications
- Active, unrestricted LPN/LVN license.
- 1+ year of clinical nursing experience.
- Experience in Primary Care, Urgent Care, or Emergency Room settings required.
- Experience with phlebotomy, immunizations, and point-of-care testing.
- Strong EMR skills; Epic experience preferred.
- Excellent communication and patient care skills.
