Compensation & Productivity
- Pay Rate: $80 per point
- Non-Visit Time (training, care conferences, PTO): $50/hour
- Mileage Reimbursement: $0.55 per mile
- Paid for mileage between the first and last visit of the day
Productivity Expectation:
Point Values:
- Follow-up / Regular Visits: 1 point
- Evaluations: 1.5 points
- Start of Care (SOC) with Evaluation: 2.5 points
- Recertifications: 1.75 points
- Resumption of Care (ROC): 1.75 points
- Discharges / OASIS Discharges: 1 point
Scheduling & Visit Expectations
- All scheduling is finalized by administration.
- Visits are scheduled and completed at the therapist’s discretion, provided they are completed within required policy timeframes.
- Any visits that cannot be accommodated must be communicated to the office the day of the scheduled visit or sooner.
- Start of Care (SOC) visits must be completed within 48 hours of referral.
- Evaluations must be completed no later than 5 days after SOC.
Responsibilities
- Provide skilled physical therapy services to patients in their homes.
- Perform comprehensive evaluations and develop individualized treatment plans.
- Implement therapeutic exercises, mobility training, and pain management interventions.
- Monitor patient progress and adjust treatment plans as needed.
- Collaborate with physicians and interdisciplinary care team members to optimize outcomes.
- Educate patients and caregivers on treatment plans, safety, and home exercise programs.
- Complete accurate and timely clinical documentation in compliance with regulatory standards.
- Participate in required training, meetings, and care conferences.
Qualifications
- Current Physical Therapist (PT) license in the state of practice.
- Experience in home health or similar clinical setting preferred.
- Strong clinical assessment and treatment planning skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and manage time effectively.
- Knowledge of Medicare regulations and home health best practices.
- Current CPR certification required.
