About the Role
The CORE CPST Provider delivers mobile, community-based behavioral health interventions to adults enrolled in Medicaid Health and Recovery Plans (HARP). CPST is a goal-directed, solution-focused service designed to improve functioning, promote recovery, and reduce barriers related to mental illness and co-occurring conditions. This role requires strong clinical judgment, documentation accuracy, person-centered planning skills, and comfort working in home and community environments.
Key Responsibilities
- Provide CPST services to adults with serious mental illness (SMI)
- Conduct comprehensive behavioral health assessments
- Develop and implement Individual Service Plans (ISPs)
Deliver evidence-based clinical interventions, including:
- Individual psychotherapy and counseling
- Motivational Interviewing (MI)
- Cognitive Behavioral Therapy (CBT)-informed strategies
- Harm reduction approaches
Support individuals with:
- Symptom stabilization
- Crisis prevention planning
- Community integration
- Independent living skill development
Coordinate care with other client providers including:
- Health Home Care Managers
- Psychiatric providers Primary care providers
- Community-based service providers
- Maintain accurate, Medicaid-compliant documentation
- Participate in clinical supervision and interdisciplinary team meetings
Qualifications
Candidates must possess one of the following New York State Licenses or permits:
- Licensed Master Social Worker (LMSW)
- Licensed Clinical Social Worker (LCSW)
- Licensed Mental Health Counselor (LMHC)
- Mental Health Counselor Limited Permit
- Licensed Marriage and Family Therapist (LMFT)
- Marriage and Family Therapist Limited Permit
- Licensed Creative Arts Therapist (LCAT)
- Creative Arts Therapist Limited Permit
- Licensed Psychologist
- Psychology Limited Permit
Additional Requirements:
- Experience working with adults with serious mental illness (SMI) and/or co-occurring substance use disorders.
- Strong clinical documentation and treatment planning skills.
- Knowledge of Medicaid-funded behavioral health services.
- Strong crisis intervention and risk assessment skills.
- Ability to work independently in community settings while maintaining professional boundaries.
- Strong communication, engagement, and documentation skills.
- Willingness to work flexible hours, including evenings or weekends as needed.
Schedule and Work Environment
- Flexible scheduling
- Day, evening, and weekend availability as needed
- Services are delivered primarily in community-based settings, including members’ homes and other appropriate locations in the community.
- This position requires local travel to meet individuals where services are most effective.

We are currently hiring in all five boroughs of New York City – Bronx, Brooklyn, Manhattan, Queens, and Staten Island.
Pay: $80.00 per hour
