Medical Social Worker – MSW | Part Time

We are seeking a Part Time Medical Social Worker to join our Care Management team. In this role, you will conduct psychosocial assessments, coordinate discharge planning, and collaborate with an interdisciplinary team to ensure patients receive the services and support they need for safe, effective transitions of care. Conduct psychosocial assessments and identify social, emotional, and safety concerns.
Specifically, you will:
- Develop and coordinate discharge plans for patients.
- Connect patients and families with community resources and support services.
- Collaborate with physicians, nurses, case managers, and other healthcare professionals.
- Communicate with insurance providers regarding authorizations and coverage.
- Maintain timely, accurate clinical documentation and case records.
- Advocate for patients to ensure appropriate care and continuity of services.
We require:
- Master's Degree in Social Work (MSW) from an accredited program.
- Current NYS LMSW or LCSW required (limited permit accepted at NHCC).
- Minimum 1-2 years of acute care or hospital social work experience preferred.
- Strong assessment, communication, documentation, and organizational skills.
- Proficiency with EMR (electronic medical records) and computer literacy.
- Screen Certification preferred.
