In-Person, Facility-Based Care — Virginia — Rural Retreat
Location: Rural Retreat, VA — On-site at partnered nursing facilities serving Rural Retreat and the surrounding Wythe County area.
About Us
Prefer NP is a rapidly growing network of advanced practice providers committed to improving access to high-quality behavioral health care for underserved populations. We are hiring licensed mental health therapists to deliver in-person psychotherapy to patients in our partnered nursing facilities across Virginia.
Our Vision
Many of the patients in the facilities we serve simply need someone to talk to — a consistent, compassionate professional who shows up in person and listens. We prioritize a patient-centered approach that brings therapy directly to residents where they live, breaking down the barriers that keep them from the support they need to thrive.
Your Role
As a licensed mental health therapist, you will provide in-person psychotherapy to patients in skilled nursing and residential care facilities. You will see patients on-site at our Rural Retreat partner facilities, building trusting relationships and delivering the day-to-day emotional support and clinical care that residents need. You will work collaboratively with our psychiatric providers and facility staff as part of a coordinated care team.
Key Responsibilities
- Provide in-person individual therapy to residents in partnered nursing and residential care facilities.
- Conduct comprehensive assessments and develop personalized treatment plans.
- Support patients with mental health conditions including depression, anxiety, trauma-related disorders, adjustment issues, grief, and cognitive-related concerns common in long-term care.
- Utilize evidence-based interventions such as CBT, DBT, trauma-informed care, and supportive therapy.
- Coordinate care with psychiatric providers, facility staff, and other healthcare professionals.
- Maintain accurate and timely documentation in the EHR system.
- Educate patients and families on mental health conditions and treatment options.
- Follow ethical guidelines and ensure compliance with HIPAA regulations.
Ideal Candidates
- Actively licensed in Virginia as one of the following: LCSW, LMFT, LPC, or Licensed Psychologist.
- 1+ years of post-license clinical experience in behavioral health.
- Willing and able to provide in-person care on-site at facility locations in Rural Retreat and surrounding communities—this is not a telehealth role.
- Comfortable working in facility-based settings (SNFs, assisted living, residential programs), including with older adults and long-term care populations.
- Strong diagnostic and clinical evaluation skills.
- Knowledge of trauma-informed care and evidence-based treatment approaches.
- Experience with electronic health records (EHR).
- Excellent communication and interpersonal skills—someone patients feel comfortable opening up to.
