Role Description:

As a clinician-owned and operated company, this in-home or hybrid opportunity involves evaluating and assessing patients' physical conditions, crafting and executing treatment plans, and diligently documenting patient progress. As a key player in our innovative approach, you'll deliver top-notch patient care, incorporating therapeutic exercises, modalities, and cutting-edge manual therapy techniques.
Salary:
- Starting at $95/hour for ongoing therapy (ongoing therapy rate to increase based on area providing services).
- Starting at $150/visit for evaluations

Requirements:
- Graduate from an accredited Physical Therapy program.
- Hold an active or eligible Arizona Physical Therapy Licensure (required upon start date).
- Maintain current CPR Certification, Valid Fingerprint Clearance Card, Article 9 Training (required upon start date, new-hires eligible to receive support to complete requirements).
Essential Functions:
- Evaluate and treat a diverse caseload, including patients with Autism, Down Syndrome, Cerebral Palsy, Cognitive Disabilities, and other chromosomal syndromes
- Document SOAP notes, progress reports, and evaluation reports using a user-friendly digital EMR system
- Showcase your ability to work independently with minimal direct supervision
