*This position requires a Master's Degree of Social Work for full competency of skills,
Job Responsibilities:
- advising family care givers, providing patient education and counseling, and making referrals for other services in the patient's home
- provide patients, and families of the patient with psycho-social support needed to cope with chronic, acute, or terminal illnesses
- assesses and treats emotional and behavioral problems related to patient illnesses in the home
- functions as a member of multi-disciplinary team, assists patients in treatment planning and advising, supporting comfort and peace at end of life
- serves as the bereavement coordinator for all in home palliative clients
Job Skills and Qualifications:
- active listening, communication, interpersonal, problem-solving, time-management, excellent judgment skills
- independent or willing to be independent in providing patients with counseling, education, and support in the home
- applicable licensure by the state of Virginia
- Graduate of a school of social work approved by the Council of Social Work Education with a master’s degree.
Job Type: Full-time
Pay: $28.00 - $32.00 per hour
