Master Level Social Worker (MSW)
This is a great opportunity for a passionate Master Level Social Worker (MSW) to join PACE Program. (Program of All-Inclusive Care for the Elderly) The MSW plans, organizes and implements social work services to participants and their families/caregivers. The Social Worker maintains a caseload of participants and works in close coordination with the Interdisciplinary Team (IDT). This individual serves as a liaison between the PACE program and the participants/families/caregivers. The Social Worker plays a key role in helping the frail elderly population to continue living independently at home. This is primarily a Center-based role, which will include occasional home visits and off-site visits.
Type: TEMP ( months +)
Status: Full Time (40 hours/week)
Schedule: Mon – Fri, 8am – 5pm (40 hours)

MINIMUM QUALIFICATIONS REQUIRED FOR THIS POSITION:
- Master’s degree in Social Work from a Council on Social Work Education (CSWE)-accredited program required
- CPR and First Aid certification required within 60 days of employment
- 1-2 years of experience conducting psychosocial assessments, care planning and case management
- A minimum of 2 years of experience working with frail or elderly populations
- Proof of a valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law
- Experience in EMR/EHR documentation preferred
- Experience working with multidisciplinary teams and medical professionals preferred
- Bilingual in English/Spanish, English/Cantonese, English/Mandarin, or English/Vietnamese preferred
