SPECIFIC RESPONSIBILITIES
- Participant Responsibilities- Assists in creating a schedule and calendar of productive on-site, in-house, and community outings to provide productive services for persons supported. Ensures the services and support offered align with individual and program specific goals.
- Assists in the day-to-day operations of the site-based program to meet the needs of the staff and participants. Responsibilities include but are not limited to daily attendance, scheduling of routes, documenting goals, completing communication sheets, attending to building clean-up, ordering & stocking supplies as needed, ensuring the maintenance of vehicle fleet.
- Maintains paperwork in compliance with SCO and/or OPWDD standards.
- Works collaboratively with Day Habilitation supervisor in planning daily/weekly/monthly events.
- Conducts on-site groups and floor support inclusive of assisting with medication administration, toileting, and lunch preparation.
- Attends life plan meetings (when supervisor is unable to attend) and conducts supervisions with staff members.
- Assists in the development and implementation of staff action plans. Ensures paperwork is contemporaneous and updated to reflect current goals and services.
- Develops and maintain positive working relationships with coworkers, volunteer sites, family members/advocates of participants, outside agency personnel, coverage staff, etc.
- Adheres to all regulations and agency policy and procedures with regard to incident reporting.
- Performs other duties as delegated, including assignments at other SCO day hab program sites if staffing shortages impede a program’s ability to operate.
- Assists in training New-Hires and Coverage Staff in SCO policies and procedures as well as day-to-day operations of the Day Hab Instructors including how to run community outings and group activities, fill out paperwork, run inter-office mail run, setup with UKG, LMS trainings etc.
QUALIFICATIONS
- Associate’s degree in human services or related field.
- Minimum of two years’ experience in the ID/DD or related human services background.
- Knowledge of OPWDD HCBS Waiver Services.
- Excellent verbal and written communication skills.
- Valid driver’s license required.
- Computer literacy required.
- Must be able to sit, stand, and ambulate for extended periods of time.
- Ability to assist with wheelchair transfers. Lifting required.
SCIP-R, AMAP, CPR, First Aid required within first 90 days of hiring.
