Full Job Description

Position Summary

The Wound Nurse provides specialized nursing oversight for residents with wounds, pressure injuries, and other skin-integrity concerns. The nurse performs comprehensive assessments, supports individualized treatment and prevention plans, monitors healing, and collaborates with the interdisciplinary care team to promote resident safety and positive clinical outcomes.

Essential Responsibilities

Complete comprehensive head-to-toe skin assessments according to facility policy and applicable clinical standards.

Evaluate and document each new admission’s skin condition promptly, identifying existing wounds, pressure injuries, bruising, rashes, surgical sites, and other areas of concern.

Conduct and coordinate weekly wound rounds with appropriate clinical team members; measure, assess, photograph when authorized by policy, and document wound status and progress.

Assess acute and chronic wounds, including pressure injuries, skin tears, vascular ulcers, diabetic ulcers, and surgical wounds.

Develop, implement, and update individualized wound-care and skin-integrity plans in collaboration with providers and the interdisciplinary team.

Perform ordered wound treatments and dressing changes using appropriate infection-prevention, clean, or sterile technique.

Monitor for changes, delayed healing, deterioration, pain, drainage, odor, or signs of infection and promptly notify the provider and nursing leadership as indicated.

Monitor skin assessments and evaluate new skin issues within 24 hours of being reported.

Implement prevention strategies such as pressure redistribution, repositioning, moisture management, nutrition support, and use of appropriate support surfaces.

Ensure wound-care orders, treatment records, care plans, and related assessments are accurate, current, and completed within required timeframes.

Communicate findings and recommendations to residents, families, providers, nursing staff, therapy, dietary services, and other members of the interdisciplinary team.

Provide education and competency support to nursing assistants and licensed nurses regarding skin observation, pressure-injury prevention, treatment procedures, and timely reporting.

Track wound trends and outcomes; participate in quality-assurance activities, audits, care conferences, and regulatory survey preparation.

Maintain adequate wound-care supplies and help evaluate products for safety, effectiveness, and appropriate use.

Protect resident dignity, privacy, and rights while following facility policies, infection-control practices, and professional nursing standards.

Serve as a member of the nurse manager team and participate in the on-call rotation.

Minimum Qualifications

Current, unrestricted registered nurse.

Current CPR/BLS certification, or completion of certification upon hire.

Clinical experience in wound care, long-term care, skilled nursing, rehabilitation, home health, or a related setting preferred.

Wound-care certification or willingness to obtain certification preferred.

Strong assessment, documentation, communication, organization, teaching, and critical-thinking skills.

Ability to work collaboratively, manage priorities, maintain confidentiality, and respond promptly to changes in resident condition.

The Wound Nurse performs other duties as assigned and practices within applicable nurse practice laws, provider orders, facility policies, and professional standards. Responsibilities may be adjusted to reflect the facility’s resident population, staffing model, and regulatory requirements.
