- Assist healthcare providers with patient care activities and clinical procedures.
- Coordinate and manage Chronic Care Management (CCM) programs to ensure patients receive comprehensive care.
- Monitor and document patient data through Remote Patient Monitoring (RPM) systems.
- Prepare patients for examinations and take vital signs.
- Maintain accurate and up-to-date patient records in compliance with healthcare regulations.
- Communicate effectively with patients regarding care plans and follow-up instructions.
- Support clinical staff with administrative tasks as needed.
- Certified Medical Assistant (CMA) or equivalent certification preferred.
- Experience with Chronic Care Management (CCM) and/or Remote Patient Monitoring (RPM) is highly desired.
- Strong knowledge of clinical procedures and patient care standards.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively in a fast-paced healthcare environment.
- Proficient with electronic health records (EHR) systems.
