RN Home Care Coordinator – Full-Time (Senior Apartment Community)
The RN Home Care Coordinator oversees the delivery of high-quality home care services for residents living in a senior apartment community. This role is responsible for assessing resident needs, developing and coordinating individualized care plans, supervising LPNs and Home Health Aides, ensuring compliance with regulatory and agency standards, and collaborating with residents, families, physicians, and community partners to promote safe, independent living. The RN provides clinical leadership, staff education, and ongoing quality oversight while supporting a compassionate, resident-centered approach to care.
Key Responsibilities:
- Coordinate and oversee home care services for residents within the senior apartment complex.
- Supervise, mentor, and evaluate LPNs and Home Health Aides to ensure quality care and compliance with agency policies.
- Perform routine supervisory visits, competency assessments, and documentation reviews.
- Monitor resident health status and revise care plans as needed.
- Ensure timely and accurate clinical documentation in accordance with state, federal, and agency regulations.
- Participate in quality improvement initiatives, infection prevention, and risk management activities.
- Provide resident, family, and staff education on disease management, medications, safety, and wellness.
- Assist with scheduling, staffing coordination, and problem-solving to ensure continuity of care.
- Respond to clinical concerns, emergencies, and resident issues as appropriate.
- Promote a culture of professionalism, teamwork, and compassionate, person-centered care.
Qualifications:
- Current Registered Nurse (RN) license in good standing.
- Minimum of 2 years of nursing experience, with home care, community health, geriatrics, or case management preferred.
- Previous supervisory experience overseeing LPNs and/or Home Health Aides preferred.
- Strong assessment, communication, leadership, and organizational skills.
- Knowledge of home care regulations, documentation standards, and quality assurance practices.
- Proficiency with electronic health records and Microsoft Office applications.
- Ability to work independently while managing multiple priorities in a fast-paced environment.
This position is ideal for an experienced RN who enjoys combining clinical expertise with leadership to support older adults in maintaining independence and quality of life within a senior housing community.
This is a full-time position with a regular schedule of Monday through Friday, 6:00 a.m. to 2:00 p.m. The position also requires one weekend rotation per month and one week of on-call coverage each month to support continuity of resident care.
Pay: $45.00 per hour
