Job Title: Coordinated Family Support (CFS) Staff
Reports To: CFS Case Manager / Supervisor
Position Overview:
The Coordinated Family Support (CFS) staff provide person-centered case coordination and family engagement services for adults with developmental and intellectual disabilities who reside in their family home. This role focuses on empowering individuals and their families to achieve independence, stability, and inclusion through education resource navigation, and individualized support planning. Under the supervision of the CFS Program Director and Case Manager Supervisor, the CFS Staff delivers field-based coordination and advocates for families. It ensures service integration consistent with Title 17, DDS CFS Guidelines, and the HCBS Final Rule.
Essential Duties & Responsibilities:
Family Support & Advocacy:
§ Conduct home and community visits to assess family needs and individual goals
§ Educate families about available Regional Center and community resources
§ Facilitate access to support including respite, behavioral services, transportation, and social recreation
§ Advocate for individual rights, self-determination, and culturally appropriate support
§ Perform other duties as assigned
Service Coordination
§ Assist families in developing and updating Individual Service Plans (ISPs)
§ Coordinate with Regional Center Service Coordinators, clinicians, and vendors to ensure consistent service delivery
§ Track progress toward identified outcomes and adjust plans as needed
Case Management & Documentation:
§ Maintain accurate case notes, contact logs, and service plans in accordance with Title 17 and U.N.I. policy
§ Document barriers to service and develop solutions with supervisors
§ Prepare quarterly and annual reports to measure family progress and satisfaction
Community Integration:
§ Support connecting families with employment, education, and social activities that foster natural support systems
§ Conduct community outreach and resource mapping within assigned service areas
§ Collaborate with local organizations, schools, and faith-based partners to enhance family resources and supports
Behavioral & Clinical Support:
§ Implement proactive support strategies under the guidance of a BCBA when behavioral issues arise
§ Support the implementation of the behavior plan and promote positive communication skills within the family unit
Compliance & Teamwork:
§ Maintain confidentiality in accordance with HIPAA and U.N.I. standards
§ Participate in team meetings, training sessions, and quality assurance reviews
§ Uphold agency values of respect, cultural humility, and inclusion in all service delivery
Qualifications:
Education & Experience:
§ Bachelor’s degree or higher in Psychology, Social Work, Human Services, or a related field (required)
§ Bilingual in Spanish (preferred)
§ Minimum of 1-2 years of experience supporting individuals with developmental disabilities or working with families in social services (required)
§ Knowledge of Regional Center systems and community resources (preferred)
Licenses & Certifications:
§ Valid California Driver’s License, reliable transportation, and proof of insurance (required)
§ DOJ / FBI fingerprint clearance required prior to hire
§ Current CPR/First Aid certification (training provided if not current)
§ Completion of U.N.I. paid mandatory training (Title 17, HIPAA, CLAS, SIR, etc.) within 90 days of hire

§ Organized and detail-oriented with solid documentation accuracy

§ Able to work independently while collaborating within a multi-disciplinary team
Working Conditions:
§ Primarily field-based role with frequent travel to family homes and community sites
§ May include occasional evenings or weekends based on family availability
§ May involve moderate physical activity and variable work environments and weather conditions

§ Work location, work schedule, and hours are subject to change based on client/company needs
Pay: From $25.00 per hour
