Heritage Healthcare of Lyndhurst is a compassionate, resident-focused skilled nursing facility dedicated to improving the quality of life for older adults. Our interdisciplinary team is committed to providing evidence-based care in a supportive and respectful environment. We are currently seeking a caring and motivated Speech-Language Pathologist (SLP) to join our therapy team.
The Speech-Language Pathologist will assess, diagnose, and treat residents with communication, cognitive-linguistic, and swallowing disorders. The SLP will work collaboratively with nursing, rehabilitation, and dietary staff to develop and implement individualized care plans that promote safety, functional communication, and the highest practical level of independence.
Conduct speech, language, cognitive-communication, and swallowing evaluations for new and existing residents
Develop and implement individualized treatment plans based on resident needs and goals
Provide therapy services for dysphagia, speech and language disorders, voice issues, memory/cognitive challenges, and communication deficits
Collaborate with interdisciplinary team members to ensure coordinated, resident-centered care
Participate in care plan meetings and family education sessions
Document evaluations, treatment sessions, progress notes, and discharge summaries in accordance with facility and regulatory standards
Provide training to staff on safe swallowing strategies and communication techniques
Maintain compliance with state, federal, and professional regulatory requirements
Master’s degree in Speech-Language Pathology from an accredited program
Current OH state SLP license (or eligibility)
Certificate of Clinical Competence (CCC-SLP) preferred; CFY candidates welcome
Experience in a skilled nursing facility or geriatric setting preferred
Strong communication, organizational, and teamwork skills
