Job Summary
We are seeking a dedicated and compassionate Part time Social Worker to join our dynamic healthcare team. Duties
- Conduct thorough patient assessments to identify social, emotional, and environmental needs
- Provide clinical counseling, including behavioral therapy, motivational interviewing, and crisis intervention
- Coordinate community resources and assist with housing as needed.
- Document all patient interactions accurately within electronic medical records (EMR) systems in compliance with HIPAA and clinical ethical guidelines
- Collaborate with interdisciplinary teams to develop individualized care plans that incorporate social work best practices
- Support hospice and palliative care patients through end-of-life discussions and bereavement support
- Facilitate individual therapy sessions tailored to diverse populations, including geriatric patients, children, individuals with disabilities, or those with developmental disabilities
- Assist with intake procedures for new patients and contribute to clinical research initiatives when applicable
- Maintain current knowledge of Medicare regulations, ICD coding standards, and hospital policies related to medical documentation and patient care
Experience
- Proven experience working as a social worker in healthcare settings such as hospitals, clinics, or hospice environments
- Strong background in mental health counseling, and behavioral health interventions
- Familiarity with medical terminology, discharge planning processes, and medical records management
- Experience working with individuals from diverse cultural backgrounds and those with complex conditions such as dementia or PTSD
- Demonstrated ability to perform diagnostic evaluations and case management effectively
- Knowledge of EMR systems for documentation purposes; prior inpatient or outpatient experience is preferred
- Certification in Basic Life Support (BLS) or CPR is required; additional certifications in clinical counseling or specialized areas like Alzheimer's or autism care are advantageous
- Ability to handle crisis situations efficiently while maintaining professionalism and compassion
