Job Overview
We are seeking a dedicated and skilled Physical Therapist specializing in Home Health to join our dynamic healthcare team. In this role, you will deliver comprehensive physiotherapy services directly within patients' homes, promoting recovery, independence, and overall well-being. The ideal candidate will possess a strong foundation in physiology and anatomy, with experience in diverse care settings including inpatient, outpatient, hospital, and community environments. Your expertise will support the development of personalized care plans, discharge planning, and patient education to ensure optimal outcomes across a broad spectrum of patient populations.
Responsibilities
- Conduct comprehensive physical therapy evaluations and develop individualized treatment plans.
- Provide therapeutic interventions to improve mobility, strength, endurance, balance, and functional ability.
- Educate patients and caregivers on home exercise programs, safety, and equipment use.
- Maintain timely and accurate documentation within the EPIC EMR system.
- Participate in interdisciplinary care planning and communicate changes in patient status promptly.
- Travel to assigned patient homes throughout the service area.
- Participate in weekend rotation schedule (once every 6 weeks).
- Ensure compliance with agency, state, and federal guidelines.
Required Qualifications
- Active and unrestricted Physical Therapist (PT) License in the state of Massachusetts
- Minimum 1 year of Home Health experience (required, no exceptions)
- CPR/BLS Certification through AHA
- Valid driver’s license and reliable transportation
- Ability to travel to multiple patient homes daily
- Proficient with electronic documentation; EPIC experience preferred

- Prior travel assignment experience
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient–family engagement abilities
