Key Responsibilities of the Physical Therapist
- Evaluate patients in their homes and develop individualized treatment plans
- Deliver skilled physical therapy interventions to improve mobility, strength, balance, and functional independence
- Monitor patient progress and modify treatment plans as needed
- Educate patients, families, and caregivers on exercises, mobility techniques, and injury prevention
- Maintain accurate clinical documentation in accordance with agency and regulatory standards
- Collaborate with physicians, nurses, occupational therapists, and other members of the healthcare team
- Ensure high standards of patient care while promoting safety and independence
Requirements of the Physical Therapist Role
- Current BLS certification
- Current Minnesota Physical Therapist State License
- Minimum 1 year of recent home health Physical Therapist experience
- Strong assessment, rehabilitation, and patient education skills
- Excellent communication, organisational, and clinical decision-making abilities
