Job Summary We are excited to offer a dynamic, autonomous Primary Care Physician opportunity focused on skilled nursing facility (SNF) care. This role is designed for dedicated medical professionals eager to deliver exceptional post-acute care to complex, geriatric, and rehabilitating patients while enjoying the ultimate flexibility to make your own schedule. As a key member of our healthcare team, you will manage your own hours to conduct daily inpatient rounding for a dedicated census of 15–20 patients, utilizing your expertise to manage acute changes in condition and chronic illnesses. - Set your own daily hours to conduct comprehensive patient evaluations through detailed history-taking and physical examinations during inpatient rounds for 15–20 patients.
- Develop and implement personalized treatment plans, including complex medication management, wound care, pain management, and disease-specific therapies tailored to a sub-acute population.
- Utilize Electronic Health Record (EHR) systems like Epic or PointClickCare to document daily patient encounters accurately and efficiently on your own timeline.
- Perform diagnostic evaluations, interpret medical imaging/labs, and direct emergency interventions or acute transfers when necessary.
- Collaborate closely with the SNF multidisciplinary team—including nursing staff, physical/occupational therapists, and case managers—to coordinate care.
- Manage complex post-acute cases, ensuring strict adherence to HIPAA regulations and quality metrics like NCQA and CMS standards.
- Lead detailed discharge planning and coordinate with outpatient providers to optimize patient outcomes and support safe post-acute care transitions.
Experience
- Post-residency experience in Internal Medicine or Family Medicine with a strong focus on inpatient rounding, geriatrics, or sub-acute care.
- Proven ability to work autonomously and manage your time effectively within a flexible scheduling model.
- Hands-on experience with EMR systems used in hospital or long-term care settings for medical records management.
- Familiarity with diagnosing and managing conditions common in sub-acute settings, including post-surgical recovery, stroke rehabilitation, and cardiac care.
- Proven ability to manage acute status changes in a facility setting, including airway management, infection control, and metabolic stabilization.
- Experience working within skilled nursing facilities, rehabilitation hospitals, or acute care environments.
- Strong communication skills for collaborating with facility staff, families, and providing exemplary patient service in diverse clinical scenarios.

Pay: $200,000.00 - $240,000.00 per year
