Position Summary:
We are seeking a dedicated and organized HHA Nurse to conduct field visits and supervisions of Home Health Aides (HHAs) in client homes across the Bronx, Westchester, Queens, Manhattan, and Brooklyn.. The ideal candidate is a licensed nurse with strong communication skills, familiarity with home care regulations, and the ability to travel within the designated areas.
Key Responsibilities:
- Perform routine HHA supervisions, annual competency evaluations, and in-service assessments
- Ensure that aides are delivering services according to care plans and agency policies
- Provide field education and retraining as needed
- Communicate findings and concerns to the clinical team and document visit notes accurately
- Monitor the health and safety of patients during visits and escalate any concerns
- Collaborate with case managers, coordinators, and patient families
- Complete documentation and reports in accordance with DOH regulations and agency protocols
Requirements:
- RN license in New York State (required)
- Minimum of 1 year of home care or field experience preferred
- Knowledge of HHA scope of practice and NY DOH home care guidelines
- Strong interpersonal and communication skills
- Comfortable with electronic documentation (e.g., EVV platforms, EMRs)
- Valid driver’s license and reliable transportation OR willingness to travel via public transit
Schedule & Compensation:
- Flexible schedule but will primarily need weekday availability.
Job Types: Full-time, Part-time
Pay: $45.00 - $65.00 per hour
